SESCA carried out a review of Trusted Assessor services across the south east.
The review found that schemes have the potential to:

– offer improved discharges which patients and social care providers are better prepared for, with more information available prior to discharge, potentially leading to better outcomes for patients and providers, and fewer readmissions
– offer a new route to collect honest and detailed feedback from social care providers which can be used to improve hospital discharge processes
– identify ways in which care providers could be supported to help ease the burden on acute hospitals
